• 16 & 17 Year Old: Passport Book/Card including renewals (A separate appointment is required for each applicant)

• NEW Adult: Passport Book/Card or Renewal of a passport issued more than 15 years ago. (A separate appointment is required for each applicant)

  Please make separate appointments for each person getting a passport.<br>You will need 2 CHECKS.<br>Passport Book: $130 Application Fee Payable to U.S. Dept. of State <br>Passport Card: $80 Application Fee Payable to U.S. Dept. of State. A passport card allows to Canada, Mexico and U.S. territories. It cannot be used for air travel.<br>$35 Execution Fee Payable to Polk City Community Library<br>Please visit travel.state.gov to make sure you have all necessary documents for your appointment.

• Passport Photo Only-No application processing (A separate appointment is required for each person)

  Please make a separate appointment for each person needing a passport photo.<br>The cost for passport photo is $15.

• Minor (Ages 0-15): Passport Book/Card including renewals (A separate appointment is required for each applicant)

• Passport Renewal (Age 21+ & Expired less than 5 years ago)-STOP! We cannot process these renewals. Please make a photo only appointment.

Please read below to see how to proceed.<br>If you can answer &quot;YES&quot; to the following questions, you DO NOT need a passport acceptance facility to process a renewal. We will be happy to take a passport photo for your renewal. Please make an appointment for a Passport Photo Only.<br>1. Do you have your current passport in your possession to submit with your renewal application?<br>2. Is your current passport undamaged other than normal wear and tear?<br>3. Was your current passport issued when you were age 16 or older?<br>4. Was your current passport issued within the last 15 years?<br>5. Was your current passport issued in your current name or can you document your name change?<br><br>If you CANNOT answer &quot;YES&quot; to all of the above questions, please make an appointment for a NEW passport.
