COVID-19 Waiver of Liability
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health
Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-toperson contact. As a result, federal, state, and local governments and federal and state health
agencies recommend social distancing and have, in many locations, prohibited the congregation of
groups of people.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily
assume the risk that I may be exposed to or infected by COVID-19 by participating in a fitness
program at the Community House of Winnetka or using the Community House of Winnetka’s
equipment and that such exposure or infection may result in personal injury, illness, permanent
disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19
at the Community House of Winnetka may result from the actions, omissions, or negligence of
myself and others, including, but not limited to, the Community House of Winnetka employees,
volunteers, and program participants and their families.
In consideration of being granted the opportunity to participate in a fitness program at the
Community House of Winnetka, to use the Community House of Winnetka’s equipment, and receive
the help, assistance and advisory services rendered by the staff of the Community House of
Winnetka during my participation, I do hereby assume the risk of contracting COVID-19 virus while
participating in a Community House of Winnetka fitness program or using its facilities and
equipment and release, discharge, and agree not to sue Community House of Winnetka employees,
agents, instructors, and independent contractors from any claim, damage, harm, infection, or
illness, including but not limited to the COVID-19 virus and related complications, arising from or
associated with my use of the Community House of Winnetka facilities or equipment.
